THE STATE OF MICHIGAN HAS CREATED A FORM THAT ALLOWS PATIENTS TO DIRECT THEIR MEDICAL TEAM NOT TO ADMINISTER OPIOIDS.

THE FORM CAN BE FILLED OUT BY A PATIENT, THEIR LEGAL GUARDIAN, OR THEIR PATIENT ADVOCATE, AND IT WILL BE INCLUDED IN THE PATIENT’S MEDICAL RECORDS.

THERE ARE EXCEPTIONS, INCLUDING WHEN A PRESCRIBER DEEMS OPIOIDS MEDICALLY NECESSARY.

OPIOID-RELATED OVERDOSE DEATHS IN MICHIGAN HAVE MORE THAN TRIPLED SINCE 2011… FROM 622 TO 2,053.

A LAW PASSED IN 2018 RESPONDING TO THE EPIDEMIC REQUIRED THE MICHIGAN DEPARTMENT OF HEALTH AND HUMAN SERVICES TO MAKE THE FORM PUBLICLY AVAILABLE ONLINE.
